Systematic Literature Review of Basal Bolus Insulin Analogue Therapy during Pregnancy

چکیده
The maternal, fetal and neonatal complications of having diabetes during pregnancy, whether pre-gestational (PGDM) or gestational (GDM), have been well-established in the literature, prompting recommendations that women achieve and maintain strict glycemic control before and during pregnancy [1]. DMassociated maternal complications include progression of retinopathy, nephropathy, hypertension, and preeclampsia. DMassociated fetal and neonatal complications include congenital malformations, stillbirth, macrosomia, shoulder dystocia, intrauterine growth restriction (IUGR) [2,3]. Poor glycemic control (glycated hemoglobin (A1c) ≥7%) in the first trimester (T1) is associated with an increased rate of major congenital malformations and spontaneous abortions. In the third trimester (T3), poor glycemic control is associated with increased rate of preterm birth, preeclampsia and perinatal mortality [2,4]. However, simply targeting A1c levels is inadequate to prevent such complications. Insulin requirements vary throughout pregnancy: increasing in early pregnancy (for mean requirement of 0.7 units/ kg), may decrease in the second half of T1, likely due to nausea and vomiting, then increase in second trimester (T2) (mean 0.8 units/kg) and T3 (mean 0.9-1.0 units/kg), and lastly, either plateaus or decreases beyond 32 weeks’ gestation [5]. Insulin dosing must be continually managed and altered on an individual basis. Therefore, stringent glycemic control during pregnancy strives to minimize glucose excursions throughout the day [6].

Diabetes in Pregnancy Supp -- Sept 2016--New Final.qxd
Control of blood glucose during pregnancy is difficult because of wide variations, ongoing hormonal changes and mood swings. The need for multiple injections, pain at the injection site, regular monitoring and skillful handling of the syringes/pen further makes insulin therapy inconvenient. Insulin pump is gaining popularity in pregnancy because it mimics the insulin delivery of a healthy human...
متن کاملDrug-related risk of severe hypoglycaemia in observational studies: a systematic review and meta-analysis
BACKGROUND Diabetes mellitus (DM) leads to multiple complications, including severe hypoglycaemia events (SHEs). SHEs can impact a patient's quality of life and compliance and may directly result in additional costs to the health care system. The aim of this review was to evaluate the risk of severe hypoglycaemia in patients with type 1 (T1) and 2 (T2) DM as observed in everyday clinical practi...
متن کاملSafety of insulin glargine use in pregnancy: a systematic review and meta-analysis.
BACKGROUND The prevalence of diabetes in women of childbearing age is increasing. As such, the number of pregnancies complicated by diabetes will inevitably increase. New insulin analogues such as the long-acting analogue insulin glargine may represent beneficial treatment options in pregnancy by ensuring that patients achieve excellent glycemic control without risk of maternal hypoglycemia. ...
متن کاملUse of 50/50 Premixed Insulin Analogs in Type 2 Diabetes: Systematic Review and Clinical Recommendations
INTRODUCTION Premixed insulin analogs represent an alternative to basal or basal-bolus insulin regimens for the treatment of type 2 diabetes (T2D). "Low-mix" formulations with a low rapid-acting to long-acting analog ratio (e.g., 25/75) are commonly used, but 50/50 formulations (Mix50) may be more appropriate for some patients. We conducted a systematic literature review to assess the efficacy ...
